"missed diagnosis lack of care discharged without knowledge"

After seeing a staff member for heavy menstrual periods they never personally examined me but arranged an ultrasound December 2023 but they never booked follow up appointment until August 2024 they said nothing was concerning on ultrasound.  The ultrasound didn't state womb measurements but stated ovary measurements. 

I asked if an updated ultrasound would be helpful they refused a further ultrasound.  Said possibly just perimenopausal will ask gp to prescribe HRT.  I explained I was still having many ongoing symptoms and ultrasound was a long time ago things can change within a year whereas their reply was your symptoms may settle once get HRT.  At this point I presumed they would review me 6 months after HRT.  I was then shocked when letter said they had discharged me back to GP care.  they never asked  me if I was happy to be discharged from their care.    GP booked extra ultrasound with womb measurements.  This recent scan has came back with an enlarged womb.  I feel the staff member I saw should have taken my concerns on board by booking a further scan with womb measurements and asking for measurements from previous scans.

I feel consultant has just pushed me off their caseload without further consideration of my symptoms.
