"Updating medical records"

I received a phone call from my partner earlier, and he informed me that the surgery refused to send to the pharmacy a prescription of 7mg of his medicine because their records only state that he is taking 3mg. It just goes to show that letters from the hospital and the consultant obviously don’t get received to this practice because as I can witness, he is taking 7mg. This isn’t the first time this medical practice has irked me. I don’t appreciate the practice calling my partner a liar. I live with the man and know exactly what medicines he takes, thank you.
