"Leaving hospital without a report"

About: Nottingham University Hospitals NHS Trust - Queen's Medical Centre Campus Urgent Community Response / Virtual Ward Service

Initially I felt alright- just followed instructions. I was admitted after two falls at home.

My part time carer/ friend followed this up and found several inaccuracies.

I’m also attending Highbury Hospital and their report indicated some serious issues with my brain. These don’t seem to have been looked at. They relate to falls and my wobbliness.

Several important sections of my form discharging me from the QMC to the Virtual Ward were were not completed.

The Nurses from the Virtual Ward were good and helpful. I believe that there were two areas for investigation mentioned by the Consultant but were not to my knowledge undertaken. One of the Virtual Ward checked and my blood were fine. May I make a suggestion that the discharge papers are also posted to check on accuracy

We’re sorry to hear there were some issues around communication and follow-up. It’s important that any concerns raised by consultants are properly looked into, and that discharge information is clear and complete. Your suggestion about posting discharge papers is a great one — it could definitely help make sure everything’s accurate and nothing gets missed.
