"Hospital discharge care plan"

About: University Hospital Monklands / Renal (Ward 1)

(as a relative),

My wife who is 75 years was admitted with a kidney stone and after a week discharged, still suffering serious pain and without an agreed aftercare plan.  In hospital she was told there would be follow up with physiotherapy and care visits, but the hospital is adamant she said it was not needed and now discharged, not their responsibility. 

She is still in great pain, particularly when needing to move for toilet, washing and other basic needs that she cannot do unaided.  I do what I can, but am physically unable to lift her.  We feel the system has let her down very badly and the hospital has failed their statutory responsibility to arrange a care package.  Coming up to the festive period, my wife is in danger of having no assistance towards her recovery from a debilitating illness.
