"Care and support at home"

My 90 year old mum had a fall at home that resulted in a fracture of a small bone in her pelvis. Following a discussion with the ED doctor mum opted to return home rather than remain in hospital. 

I made a call to the older people’s team late Friday afternoon and was advised that an email would be forwarded to the team manger. The social work manager (Sloan) called to discuss what support we needed in addition to the current care package. He advised that he would make referrals to Community OT and Physio and request additional care slots. A couple days later I received a call from mum’s named social worker (Emma) who had returned from leave the previous day to say additional slots would commence the following day.

Later that morning Occupational Therapist (Joanne) called and visited. Joanne carried out a comprehensive assessment. She was perceptive, thorough, professional and kind, arranging to return the following day to assess mum in showering. 

Mum’s regular Trust care workers and their supervisor (Marie) have been extremely supportive and kind which we greatly appreciate.
