(as the patient),

I have gradually lost weight over the past few years to assist with diabetes control. Following a severe COPD episode I had an emergency admission to hospital and was very unwell. On discharge I felt weak and my weight had dropped to an all time low. On discharge the dietetic team took great care of me providing support and advice along with very useful information to help me maintain a fit and healthy weight.
