Received a visit from a Key Worker from the Adult Community Care Service on first day at home, after being discharged from hospital after a three week stay.On her visit, we decided as a couple, we didn’t need help with bathing or dressing as we could cope with that ourselves. Arrangements for home accessories eg. Commode, two raised toilet seats, special seating chair and cushion, by the Red Cross was excellently sorted.
Agreed goals by Key Worker were agreed, and a Physiotherapist from the team would contact us for a home visit. That is the only one time we saw a Key Worker, so our goals were not pursued. One visit from physiotherapist happened, but due to no pressure allowed on foot, any work by physio was limited, so therefore no more visits were arranged. Two phone calls were made to the Community Nursing Service for updates on our part, but no return of calls were received. After six weeks phoned the Community Nursing Service to inform them the plaster cast had now been removed and a replacement boot put on.
What service from the physio could we receive? Doubt now to service as we were past our six week remit but the physio would ring for a conversation. A tentative appointment with the physio was agreed.
