"Not received discharge paperwork"

About: City Hospital / CAMHS

(as the patient),

I had been attending CAMHS in 2024- 2025. I Was discharged back in October 2025, before being discharged I was told that a full report/ proof of diagnoses would be sent to me within a month.

It has now been 8 months since and I am still awaiting this report. I have phoned CAMHS in Aberdeen numerous times to try and sort the issue, but this has not helped. I have tried emailing the Dr I was seeing, who told me my report would be sent within a week. That is now two weeks ago.

I am moving abroad soon (which I have told them this) and explained that I really need the report ASAP. I really liked the doctor I was seeing, so I really don't want to submit a formal complaint, but obviously waiting 8 months for a report is just unacceptable.

Dear Princess K,

I am very sorry to hear about your experience and the frustration this has caused. You are absolutely right that a discharge letter should have been sent to you within a month, and I am sorry that this did not happen.
We have recently updated our clinical governance guidance for clinical staff to ensure they are clear on the documentation expected of them. Unfortunately, this was not followed on this occasion, and I apologise on behalf of CAMHS.
I appreciate that you have already made attempts to chase this up, and I am sorry that you have had to do so. If you could please email gram.camhs@nhs.scot for the attention of myself, Siobhan Crawford, Service Manager, I will ensure this is actioned as a matter of priority.
