"Social work package of care"

My dad is currently in Hairmyers suffering from Delirium, added to this he also has stage 4 lung cancer with bone metastasis. His prognosis is terminal with the only treatment available there to make him comfortable or as was described better quality of life. 

He has been in Hairmyers for the last 7 weeks and while his overall care by the nurses has been exceptional on the whole the doctors and social work department are letting him down. 

It has taken so far 6 weeks in obtaining a package of care, the doctors don't seem interested in treating his delirium, at one point they requested a meeting to tell us he had cancer and his delirium might be caused by a heartburn medication.

After 1 week the doctor wanted to discharge him without this care package and myself and wife have had to fight for him to get the additional support he needs. We have been advised the condition he finds himself in requires 24 hour care. My dad has paid into the system for over 40 years and when he does need support he is also told why don't you pay more money and source your own care.

My dad is currently desperate to be discharged and he is living the last weeks he has rotting in the hospital as they cannot secure this care package for him, the social work team have told us we can secure our own and pay for it ourselves if we want! 

Again, I cannot stress enough this is the social work and not the hospital in delaying this.
